CAARN: Bridging communities and researchers for healthy aging.
The Community-Academic Aging Research Network (CAARN) brings together community and health system partners with academic researchers to conduct research to improve the health of older adults and decrease health disparities.
Working daily to improve the health of our community.
Following the “Pathways Community HUB” model, UniteWI trains and develops Community Health Workers (CHW), who are trusted and acquainted service providers of the communities they serve. These CHW's utilize state-of-the-art data collection software to address the social determinants of health, to improve health outcomes. Through community partnership UniteWI is the "Bridge" to assure optimal & efficient outcomes for both community members & service providers.
